IHMMP`s parent company – Centene Corporation – recently announced that the name IHMMP would cease as of June 26, 2020 and that the new renamed name of this plan would now be MeridianTotal. More than 20,000 CHC participants in southeastern Pennsylvania will witness a change of service coordinator after two (2) of the three (3) CHC plans announced that they will not renew contracts with most external coordinating agencies with effect from January 1, 2020. Keystone First CHC terminates contracts with 20 of its 23 external service coordinating agencies and UPMC CHC terminates the contract for one of its two other external service coordination agencies. Bright Health, a Medicare advantage plan, is brand new and will launch in Illinois on 01.01.2020. ChC will complete its three-phase deployment on January 1, 2020, when the program will begin in the Northwest, Northeast and Lehigh/Capital regions (“Phase 3”). As soon as this happens, the CHC will be national for the first time. Since January 1, 2017, CHC has been working with Blue Cross and Blue Shield of Illinois under a joint upside-only commercial ACO savings agreement, based on a value-based incentive model, which rewards CHC for achieving quality, performance and cost objectives. The ACO agreement uses the existing blue cross and blue shield PPO agreement from a supplier or group and is not a new plan or HMO. More than 140,000 people in the third phase regions will move to CHC on January 1, 2020. As of mid-November, most of them (61%) had not yet opted for a chc managed care plan. The state automatically admitted these people to one of three CHC plans – namely AmeriHealth Caritas, PA Health and Wellness or UPMC. However, most participants who participated in an OLTL waiver and received long-term medicaid-funded services and assistance at home selected an HCC plan and were not automatically assigned. Relevant participants should have already received a letter from their CHC plan explaining the next steps.

Participants can choose a new service coordinator who works directly for their CHC plan or select one from the external service coordination agencies that are still contractually agreed with their CHC plan. Keystone First CHC`s other external agencies are Liberty Community Connections, involved in care coordination and disability united services; and for UPMC CHC, it`s all capabilities. This document describes the functions of the HCC and aims to gather feedback from stakeholders. The Commonwealth invites participants, interest representatives, providers, management care organizations, care coordinating agencies, legislators, family members and other interested members of the public to respond to this document.
